Job Description:

Schedule: Mon-Fri 8:00AM - 5:00PM Contract: 3/02/2020 - 5/01/2020 Responsibilities: Under the supervision of a Registered Nurse, conducts telephonic and/or in-person visits to members to provide health coaching, structured assessments, and may develop appropriate care plans. Responsible for collaborating with healthcare providers and members to optimize member benefits and to promote effective use of resources. Provides self-care management education to members regarding lifestyle changes and things that they can do to better manage their health conditions. Assists members to change behaviors, to eliminate gaps in care and to establish a medical home and to locate and access interpersonal, family, and community resources that will make it easier to manage their health. Required Experience: Current licensure as a Licensed Vocational Nurse (LVN). At least two years of related experience with Medicaid population or case management experience in a home health or health plan setting. Bilingual: English/Spanish. Previous STAR+PLUS Medicaid experience preferred. Prior experience working with community resource organizations preferred
