Job Description:

Bachelor's Degree in Social Work from a School of Social Work accredited by the Council on Social Work Education. Prefer experience in a medical setting. Must be Licensed through the State of Texas. Maintain ongoing continuing education units.

Knowledge of Services provided through the different agencies in the community, eligibility criteria for Medicare and Medicaid, and procedures for Nursing Home placement. Skilled in good oral and written communication skills. Ability to read and interpret standards for the different agencies and sympathetically deal with daily crisis situations.

Job Summary

The Social Worker will assist patients, families, and significant others in coping with the psycho social aspects of illness and its treatment. A psycho social assessment will evaluate social, psycho social, emotional, financial, and environmental needs of patients. This position works closely with Utilization Review staff for discharge planning and psycho social needs of patients.

POSITION STANDARDS:

  1. Serves as a public liaison with various community services

  2. Responsible for establishing rapport and maintaining an effective system of communication with other departments within the hospital and medical staff.

  3. Responsible for the interrelationships formed between departments within the hospital and

outside the facility.

a. Contact with supporting agencies are to be formed initially by this individual and

support of same sustained by this individual.

  1. Perform social work activities for Cecil R. Bomar Rehabilitation Center as well as in the acute setting.

  2. Supervise social work interns placed at Memorial Hospital

a. Provide weekly MSW supervision to social work students.

b. Coordinate and interview students interns prior to placement with the hospital

c. Liaison to Stephen F. Austin State University in placing social work interns.

  1. Participated in interdisciplinary committees as requested to represent social services.

  2. Educate other employees of the hospital as to the scope of social services and in doing so provide professional growth in other departments.

  3. Assess patients and families to determine overall needs, then addresses these needs with immediate and positive action, i.e., medication assistance, transportation, community resources.

  4. Participates in interdisciplinary meetings, including utilization review, social services, and the hospital-wide discharge planning program. Social services is a key player in this program in assisting patients and families with needs upon discharge, i.e., nursing home placement, durable medical equipment.

  5. Assess financial needs and make appropriate referrals, dependent upon patient needs and income. Assist patients in completion of forms, if necessary, to get reimbursement for the patient and hospital.

  6. Evaluates all high risk patients, identified as head injury, motor vehicle accidents, HIV/AIDS, any patient. This evaluation can take place during Discharge Planning.

  7. Participates in Quality Assurance programs and any other interdisciplinary groups within the hospital at the request of the Department Director.

DEPARTMENTAL OR UNIT SPECIFIC:

  1. Recognizes patients' psychosocial development and modifies approaches based on findings.

  2. Recognize signs of abuse and/or neglect. Notifies appropriate protective services.

  3. Develop discharge plan based on specific factors.

  4. Involves family or significant others in decision making.

  5. Recognizes potential lifestyle changes related to illness.

  6. Participates in department Quality Improvement Plan.
