Under direction of the Administrator and supervision of Regional
Reimbursement Coordinator, the Clinical Case Manager (CCM) is
responsible for management of all processes involved with
assessment, planning, facilitation, coordination, evaluation and
advocacy for service options to meet individual resident needs. The
CCM will promote quality and cost-effective outcomes while adhering
to Federal and State guidelines, and in accordance with company’s
policies and procedures.
ESSENTIAL JOB FUNCTIONS:
•Manages the RAI Process from resident admission to
• Maintains clinical compliance, receiving appropriate funding
from Medicare, Medicaid and Managed Care pay sources.
•Completes MDS and all pertinent forms required in a timely,
accurate and case mix optimized manner.
•Develops, with input from the interdisciplinary team members,
individualized care plans
•Coordinates resident assessments and services by other clinical
• Provides timely information for all managed care clinical
authorization and justifications.
•Completes assigned duties outlined on “Role of the Clinical
Case Manager” procedure form.
•Provides training and support to nurses and nurse aides on
Activities of Daily Living (ADL)
•Manages the daily PPS/OBRA case management meetings and weekly
level of care meetings
•Attends daily start up, stand up, and weekly standards of care
•Participates in weekly standards of care meetings, monthly
quality assurance, PI and triple check meetings
•Attends training, meetings and seminars when necessary
SUMMARY OF QUALIFICATIONS:
•Current Texas Registered Nurse (RN)
•Excellent communication skills
•Proficient computer skills: Microsoft office and ability to
learn new applications.
•Minimum Data Set and Care Plan experience preferred.
•Knowledge of Medicare and Medicaid programs preferred.
•Minimum of one (1) year long term care experience, preferably
with some supervisory responsibility.
•Must be committed to regular attendance and working irregular
hours, including evenings and weekends as necessary.
